Our staff works together as a team to provide the best possible healthcare for your pet. You are welcome to download the New Patient Registration Form prior to your pet’s appointment and complete it at your leisure in the convenience of your own home. To schedule an appointment, please call us at 309-444-2311 and we will find a time that is convenient for you. If you have any medical history on your pet, please bring it with you to the appointment or have it faxed to us prior to the appointment as this will help us in determining your pet’s needs.
